Which mask for what

The laryngeal mask is placed in the pharynx and seals against the entrance to the larynx without passing the vocal cords. That makes it faster to insert than an endotracheal tube and usable by someone who does not intubate routinely — which is why the ERC recommends a supraglottic airway over repeated intubation attempts in out-of-hospital cardiac arrest.

  • i-gel — seals with a moulded gel cuff instead of an inflatable cuff, so no cuff has to be inflated or checked. Size 4 has a gastric channel for a gastric tube, which empties the stomach and reduces the risk of regurgitation during longer use.
  • Ambu Aura-i — a cuffed mask with a built-in bite block and a curved airway channel intended for fibreoptic intubation through the mask. The right choice where the mask may need to become a route to a definitive airway.

Size follows body weight: size 4 for roughly 50–90 kg, size 5 for the heaviest adults. If you carry only one size in the bag it should be 4 — it covers most adults.

What the mask does not do

No laryngeal mask seals against the trachea the way a cuffed tube does. Aspiration protection is incomplete, and with a full stomach, high airway pressures or a need for prolonged ventilation, endotracheal intubation is still the right answer. Which size of laryngeal mask should I choose?

Size is chosen by the patient's body weight, not height. Size 4 covers roughly 50–90 kg and fits most adults; size 5 is for the heaviest. If the bag has room for only one size, size 4 is the choice. Always check that the chest rises and that capnography shows a normal waveform after insertion.

What is the difference between i-gel and a cuffed laryngeal mask?

i-gel seals with a pre-formed gel cuff that moulds to the pharynx with body heat — there is no cuff to inflate and no cuff pressure to check. A cuffed mask such as the Ambu Aura-i seals with an inflatable cuff instead, which allows the seal to be adjusted but requires the pressure to be kept low so the mucosa is not damaged.

Does a laryngeal mask protect against aspiration?

Not completely. It seals against the entrance to the larynx but not inside the trachea, so stomach contents can pass by. With a full stomach, high airway pressures or a long ventilation time, endotracheal intubation is the right choice. Size 4 of the i-gel has a gastric channel for a gastric tube, which reduces the risk but does not remove it.
