Spinal boards

A spine board is used today mainly to move and evacuate a patient, not as a surface for the whole transport — time on a hard board should be kept short.

What the board is good at

The spine board is a transfer tool. It lets a patient be lifted out of a vehicle, carried over rough ground or moved between stretcher and examination table with control of the back and neck maintained. The board is radiolucent, so imaging can be done without lifting the patient off.

What it is not good at is being lain on. A hard surface causes pressure injuries faster than you would think, makes breathing harder and hurts a patient who is already in pain — so time on the board should be kept short, and transport should be on a vacuum mattress or a stretcher where possible.

Securing in the right order

Secure the torso first, the head last. The spider strap spreads the hold across chest, pelvis and legs; SpeedBlocks lock the head to the board once the body is already still. The opposite order loads the cervical spine when the body is adjusted afterwards.

The roll onto the board is done together by several people, with one person holding the head and counting. Check that the patient can breathe freely once secured, and that the straps do not lie across the abdomen.

Frequently asked questions about spinal boards

Should everyone with a suspected spinal injury lie on a spine board?

No. The view has changed: the aim is to limit movement, not to strap the patient to a hard board. The board is used to move and evacuate, while a vacuum mattress or a stretcher is better during the transport itself. Time on the board should be kept as short as possible.

What do SpeedBlocks and the spider strap do?

SpeedBlocks is a head immobiliser that is adjusted for width and locks the head to the board. The spider strap is a strap system that spreads the securing across chest, pelvis and legs. Always secure the torso first and the head last — otherwise the cervical spine is loaded when the body is adjusted afterwards.

Does our organisation need a spine board?

It belongs where injured people are actually moved: ambulance services, fire and rescue services, industry with its own works fire brigade and larger events. For workplaces, schools and clubs, splints, a cervical collar and a rescue sheet are enough. Equipment that is never used also rarely gets practised with.
